Integrated Health Program

The  DeKalb Community Service Board Integrated Health Program (IHP) is aimed at improving the health and quality of life of DeKalb CSB patients through integrated care.

What is Integrative Care?

Integrative Care refers to our taking care of both your mental health care and your primary care together, in one location, so that your health can be managed in the best way possible.

Together your therapist, nurse practitioner and peer navigator will work together to identify, monitor and manage your primary health care needs.

 

Who Can Sign Up?

All clients receiving mental health care from DeKalb CSB are eligible to receive primary care through the Integrative Health Program.  Ask any DeKalb CSB staff member for more information. After a brief interview, your no-cost health screening will be scheduled.

 

Where are the Integrated Health Clinics Located?

The IHP is currently available at two DeKalb CSB locations:

Winn Way Mental Health Center
445 Winn Way
Decatur, GA 30030
Clifton Springs Mental Health Facility
3110 Clifton Springs Road
Decatur, GA 30030
